Tirzepatide label steps expressed in syringe units
The FDA-approved product is a pen or single-dose device dosed in mg, so this conversion only applies when a vial is drawn with a U-100 syringe. Each mg step below is copied from the Tirzepatide 15 mg schedule; the units are arithmetic at three example concentrations.
| Step | mg | at 5 mg/mL | at 10 mg/mL | at 20 mg/mL |
|---|---|---|---|---|
| Weeks 1–4 | 2.5 | 50 u | 25 u | 12.5 u |
| Weeks 5–8 | 5 | 100 u | 50 u | 25 u |
| Next step | 7.5 | over 1 mL | 75 u | 37.5 u |
| Next step | 10 | over 1 mL | 100 u | 50 u |
| Next step | 12.5 | over 1 mL | over 1 mL | 62.5 u |
| Maximum | 15 | over 1 mL | over 1 mL | 75 u |
Worked example
Example: 5 mg vial + 2.5 mL diluent → 5 ÷ 2.5 = 2 mg/mL (2,000 mcg/mL).
How it works
1. Note the vial content
Find the amount of peptide in the vial in mg (check net peptide content on the COA if reported).
2. Note the diluent volume
Record the volume of diluent added, in mL.
3. Divide
Concentration in mg/mL = vial mg ÷ diluent mL. Multiply by 1,000 for mcg/mL.
Frequently asked questions
How do I calculate peptide concentration after reconstitution?
Divide the vial content in mg by the volume of diluent in mL. A 10 mg vial with 2 mL is 5 mg/mL.
Does more water change the amount of peptide?
No. Adding more diluent lowers the concentration; the total amount in the vial stays the same.
Should I use the label mg or the COA net content?
If a COA reports net peptide content, that figure is the more accurate basis for arithmetic.
Is this a dosing tool?
No. It is arithmetic for laboratory work and does not recommend amounts for people.
Is Tirzepatide measured in units?
FDA-approved Tirzepatide pens are dosed in mg. Syringe units only apply when a vial (compounded or research product) is drawn with a U-100 syringe, and depend on its concentration.
