Brand-name or compounded: why it matters

Brand-name semaglutide (Ozempic, Wegovy, Rybelsus) and tirzepatide (Mounjaro, Zepbound) are FDA-approved: each batch is made under drug manufacturing rules and the dose in the pen is verified. Compounded versions are mixed by a pharmacy and are not FDA-approved. Pharmacies were allowed to copy these drugs widely only while they were on FDA's shortage list; FDA declared the tirzepatide shortage resolved in late 2024 and semaglutide's in February 2025. Oral drops, lozenges and "oral tirzepatide" have no approved equivalent at all.

For peptides such as sermorelin, BPC-157 or MOTS-c there is no currently marketed approved product, so any prescription is compounded. FDA lists BPC-157 among bulk substances that may present significant safety risks. Background: are peptides legal?

Five checks before you sign up

  1. Is the prescriber licensed in your state? Look the name up on your state medical board.
  2. Does the page say whether the drug is brand-name or compounded? If it is compounded, which pharmacy makes it, and is it a 503A or 503B facility?
  3. Is the price for the membership, the medication, or both?
  4. Can the prescription go to your own pharmacy, so insurance can apply?
  5. Is there a documented process for side effects and dose changes?
